How to use a US medical invoice and superbill template
A US medical template should work for patient billing and reimbursement support. It needs more structure than a standard services invoice because patients and insurers may need provider NPI, service date, diagnosis, procedure codes, payments and balance due.
Keep one clean master file for your practice, duplicate it for each patient or visit, and save the issued copy with the payment record according to your practice policies.
Fields to keep in the template
- Provider or practice name, address, phone and NPI.
- Patient name, date of birth and optional patient account number.
- Service date, invoice date and place of service or telehealth note where useful.
- Diagnosis code and CPT or HCPCS service code when needed for reimbursement.
- Charge, patient payment, insurance payment, adjustment, balance due and payment terms.
- Insurance company, member ID, authorization or claim reference when available.
NPI and reimbursement details
CMS describes the National Provider Identifier as a unique identifier for covered health care providers used in HIPAA administrative and financial transactions. A superbill template should leave room for that identifier when the patient will submit an out-of-network claim.
Use codes only when they match the documented service. The template should organize billing information, not encourage unsupported coding.
Privacy and minimum necessary detail
