US insurance billing is not a retail sales invoice. The policyholder needs to connect the amount due to a specific policy, insurer, term and transaction, while the producer needs clean separation between admitted premium, fees, instalments, mid-term changes and any surplus-lines taxes or stamping fees.
What a US insurance invoice should include
A useful agency-bill invoice makes the policy context visible. NIPR describes a producer as someone required to be licensed under state law to sell, solicit or negotiate insurance, and state licensing requirements vary, so include the producer or agency license detail your state and customer expect.
- Agency, broker or producer name, address, phone, email and payment instructions.
- Producer license number, agency license number or state registration reference where applicable.
- Insured or policyholder name, billing address and client account reference.
- Carrier, policy number, line of coverage, coverage period and effective date.
- Transaction type: new business, renewal, endorsement, cancellation, instalment or audit premium.
- Premium, broker fee, policy fee, agency fee, finance charge, taxes, stamping fee and return premium separately.
- Due date, cancellation warning, remittance instructions and client-money or trust-account notes where relevant.
Premiums, fees and return premiums
Keep the carrier premium separate from broker or agency fees. For a mid-term endorsement, show the effective date and pro-rated additional or return premium. For a cancellation, show the return premium as a negative line or issue a credit-style document.
Do not hide finance charges, instalment fees or agency fees inside the premium. Policyholders and commercial insureds often need to reconcile those categories separately.
Surplus lines taxes and stamping fees
Admitted-market premiums are generally not retail sales-tax charges. Surplus lines placements are different: state surplus-lines premium tax, stamping fees and broker-fee treatment vary by state and placement.
