How to use a US dental invoice and superbill template
A dental invoice template should support patient billing, private-pay receipts, dental superbills and reimbursement paperwork. It needs provider NPI, treatment date, procedure code, tooth number, insurance reference, payments and balance fields.
Use the template as a master file for cleanings, exams, X-rays, fillings, crowns, orthodontic visits, implants, dentures, oral surgery and lab-fee billing. Save issued copies according to your practice policies.
Fields to keep in the template

CMS describes the NPI as the unique identifier used by covered health care providers in HIPAA administrative and financial transactions. Keep NPI available on the template when the document may support an insurance or out-of-network claim.
Use procedure codes, tooth numbers and descriptions that match the documented treatment. Separate treatment stages, appliances, lab fees and cosmetic items so the patient and plan administrator can understand the bill.
Privacy and HSA/FSA support
HHS minimum-necessary guidance says protected health information should be limited to what is needed for the purpose. For a dental invoice, concise billing detail is usually better than clinical narrative.
IRS Publication 502 includes many dental disease prevention and treatment expenses, while teeth whitening is not includible. Keep cosmetic and treatment charges separate.
