How to use a Belgian dental invoice template

A Belgian dental template should identify the practice, provider, patient, treatment date and tooth detail without becoming a full chart note. Keep therapeutic care, cosmetic services, products and admin charges separate.

Fields to keep in the template

  • Dental practice name, address, contact details and payment instructions.
  • Treating dentist or provider and INAMI/RIZIV number where relevant.
  • Patient, service date, invoice date and optional insurer or mutuality reference.
  • Tooth number, treatment code, nomenclature code or concise procedure description.
  • Payments received, reimbursement notes, balance due and VAT status.

Therapeutic VAT exemption

FPS Finance lists non-aesthetic dental activities among common Article 44 VAT exemptions. Since 2022, the medical exemption is tied to therapeutic purpose, so purely aesthetic services and non-therapeutic charges should be reviewed separately.

INAMI/RIZIV and eAttest

INAMI/RIZIV publishes dental nomenclature and tariff information, and eAttest became compulsory for doctors and dentists from 1 September 2025 except in limited situations. The template supports billing records; it does not replace required attestation workflows.
