How to use a UK private practice invoice template

The UK VAT position depends on who provides the service and why it is supplied. HMRC Notice 701/57 links exemption to statutory registration and to services whose primary purpose is protecting, maintaining or restoring health.
Fields to keep in the template

- Patient name and optional patient ID.
