Job Application Form Template (Malta)
Updated on 13 August 2026
A job application form collects a candidate's basic details, work history, and certification before an interview — it's an employer's intake form, not a CV the candidate writes. What an employer can and can't ask on this form is restricted by Maltese employment and equality legislation.
The form below is the editor: type straight into the highlighted blanks. A candidate can complete the form and sign the certification on screen, or you can download a clean Word or PDF file to hand out — no sign-up, no watermark.

Disability discrimination in hiring is restricted by its own Act
The Equal Opportunities (Persons with Disability) Act prohibits discrimination on the grounds of disability in employment, including in hiring decisions, and requires reasonable accommodation for workers with disabilities. The Employment and Industrial Relations Act sets Malta's general employment framework alongside it. Together, they mean this form shouldn't ask a general health question; if a role genuinely has a physical requirement, frame it around that specific requirement rather than a candidate's health or disability in general.
Gender equality sits under a separate Act
The Equality for Men and Women Act addresses discrimination on the grounds of sex and gender separately from the disability-specific legislation — together with the Employment and Industrial Relations Act and EU-derived data-protection law, they form the framework this template is built to respect.
Keep intake separate from screening
This application collects the candidate's own information for the position they're applying to. If your hiring process also runs a background check, that consent and disclosure belongs in its own, stand-alone document, not folded into this application. Mixing screening consent into an application form is a real defect in some widely used free templates.

Disability discrimination in employment is prohibited
The Equal Opportunities (Persons with Disability) Act prohibits discrimination on the grounds of disability in hiring, firing and other employment decisions, and requires reasonable accommodation.
Laws of Malta — Equal Opportunities (Persons with Disability) Act (Cap. 413)The Employment and Industrial Relations Act sets the general framework
Malta's general employment protections, alongside sector-specific rules, sit under the Employment and Industrial Relations Act — confirm both this and the disability-specific Act when assessing what can be asked of a candidate.
Gender equality is addressed separately
The Equality for Men and Women Act addresses sex and gender discrimination, distinct from the disability-specific legislation.
How to use this template
- Fill in the applicant and position details. Type into the highlighted blanks: the candidate's contact details, the position applied for, and availability.
- Add education and employment history. Fill in the education background and prior employment blanks.
- Avoid disability-related questions unrelated to the role. Only frame questions around genuine, specific requirements of the role, not a candidate's health or disability in general.
- Keep screening separate. If you also run a background check, use a dedicated consent form for that — don't add it to this application.
- Have the applicant certify and sign, then download. The applicant signs the certification on the signature line at the foot of the form. Download it as a Word or PDF file for your records.
Frequently asked questions
Is this the same as a CV?
No — this is an employer's intake form for the candidate to fill out, distinct from a CV the candidate writes and brings themselves.
Can I ask about the candidate's health or disability?
Only in relation to a genuine, specific requirement of the role — the Equal Opportunities (Persons with Disability) Act prohibits discrimination on the grounds of disability and requires reasonable accommodation.
Which Act covers general employment protections in Malta?
The Employment and Industrial Relations Act sets the general framework; the Equal Opportunities (Persons with Disability) Act and the Equality for Men and Women Act address disability and gender discrimination specifically.
What if the role does need a background check?
Keep that consent and disclosure as a separate, stand-alone document from this application form.
Do I need a certification and signature section?
Yes — it's standard practice for the candidate to certify the information is accurate before submitting the application.
Does data-protection law affect what I collect on this form?
Yes — as an EU member state, Malta applies GDPR, so only collect information that's genuinely relevant to assessing the candidate for the role.
