Private dentist invoice generator for the UK
Create a UK private dental invoice with patient details, treatment date, GDC registration number, tooth notation, procedure description, private fee, insurer reference and VAT treatment. Use it for private dentistry, orthodontics, hygienist appointments, dental plans and insurance reimbursement paperwork.
This page is for private, itemised dental billing. NHS dental charges in England are grouped into bands for a course of treatment, while private treatment is normally itemised by procedure, tooth, appointment date, lab fee or cosmetic service.
What a UK private dental invoice should include
A private dental invoice should help the patient, insurer or dental-plan administrator connect the amount charged to the treatment actually provided. Include enough clinical billing detail for reimbursement, but do not turn the invoice into a full clinical record.
- Dental practice name, address, contact details and VAT number if relevant.
- Dentist, hygienist, therapist, technician or orthodontic therapist name and GDC registration number.
- Patient name, optional date of birth and insurer or plan reference.
- Treatment date, tooth number or notation and procedure description.
- Separate lines for examination, X-ray, hygiene, filling, crown, extraction, orthodontics, lab fee or whitening.
- Payment received, balance due, claim reference and brief aftercare or plan notes.
VAT-exempt dental care and taxable cosmetic work
HMRC VAT Notice 701/57 says dentists and dental care professionals may exempt supplies of dental care and treatment, drugs or appliances provided in dental care, and dental prostheses such as dentures, crowns, bridges and plates.
The same notice says purely cosmetic services are standard-rated. If an invoice contains exempt treatment and a taxable cosmetic item such as appearance-only whitening, keep the lines separate and mark the VAT treatment line by line.
Private invoices versus NHS dental charges
NHS England dental treatment is grouped into bands. The NHS page says patients pay the highest band in a mixed course of treatment, rather than an itemised fee for each procedure. Current England charges shown there are Band 1 GBP 27.90, Band 2 GBP 76.60, Band 3 GBP 332.10 and urgent treatment GBP 27.90.
Use this generator for private invoices and patient reimbursement documents, not as a replacement for NHS practice-management or NHS claims software.
Tooth numbers, lab fees and insurance claims
Use the tooth/code field for the notation your practice, plan or insurer expects. For crowns, bridges, dentures, implants or aligner stages, separate the clinical fee, lab fee and cosmetic or non-clinical elements where that helps explain the bill.
If the patient will claim insurance, include treatment date, provider registration, procedure description, tooth number and the amount paid or due.
Common UK dental invoice mistakes
- Using a generic medical receipt with no GDC registration or tooth/procedure detail.
- Mixing private treatment with NHS banded charges in a way the patient cannot understand.
- Marking cosmetic-only treatment as VAT-exempt without checking its purpose.
- Including excessive clinical notes on a document that may go to an insurer or employer.
- Not separating lab fees, treatment stages or payments already received.
