What a Canadian veterinary invoice should include

A veterinary invoice should identify the owner as the payer and the animal as the patient. Include enough detail for the owner, practice-management system and pet-insurance claim without turning the invoice into the full clinical record.

  • Practice, animal hospital or mobile vet name, business number, address, phone and payment instructions.
  • Attending veterinarian and provincial licence or practice reference where relevant.
  • Owner name, billing details and optional pet-insurance policy or claim reference.
  • Patient name, species, breed, sex, age, weight and microchip number where known.
  • Treatment date, visit type, exam, vaccination, diagnostics, procedure, hospitalization or follow-up.
  • Consultation, surgery, lab work, imaging, medication, vaccines, microchipping and disposal fees separately.
  • Retail goods, prescription diets, grooming, boarding and pet supplies separate from veterinary treatment.
  • GST/HST, payment status, discharge or insurance notes and balance due.

GST/HST and itemized treatment lines
