What a Canadian medical invoice should include
- Practice, clinic or provider name, address and contact details.
- Provider licence, college registration or professional number where relevant.
- Patient name, service date, invoice date and optional patient account reference.
- Description of consultation, treatment, therapy, report, assessment or allied-health service.
- Insurer, policy, authorization, claim reference or private reimbursement notes when needed.
- Payment received, balance due and GST/HST exempt or taxable treatment by line item.
GST/HST exemption and taxable charges
CRA supply-status guidance treats many health, medical and dental services by licensed physicians or dentists for medical reasons as exempt. Exempt supplies do not charge or collect GST/HST.
Do not assume every health-adjacent charge is exempt. Cosmetic services, administrative reports, products, supplements, room rentals or non-treatment fees may need separate review.
Insurance, reimbursement and privacy
Patients may need a clear invoice or receipt for private insurance, HSA-style spending accounts, employer reimbursement or personal records. Include the date, provider, service description and amount paid, but avoid unnecessary clinical detail.
Common Canadian medical invoice mistakes
- Treating every clinic charge as exempt without checking the supply.
- Leaving out the actual service date when the payer needs it for reimbursement.
- Omitting provider registration or licence details when the insurer asks for them.
