Why the Canada version is different
The Jotform source treats the document as a generic PDF. For Canada, that is not enough. The local version has to ask which regulator, registry, publisher, venue, client file, image release or booking regime is actually involved. That is why this page changes the form fields and the checklist rather than just changing spelling and currency.
The template also avoids two common defects in free forms: a signature block that makes a proposal look binding, and a broad waiver or non-refundable fee that ignores local fairness rules. Where a later official filing, engagement letter, publisher contract, land registry application or boxing approval is needed, the document says so.
